City of Whitehorse

Enhancing a person-centred approach in service delivery, including strengthening systems and collaborative practices.

City of Knox

Workforce development strategy, including policy and procedural development.

City of Casey

Active Service Model Plan, including staff facilitation to develop care plan tools, practices and pathways.

City of Moreland

Partnership project which included a workforce development plan between three community organisations.

Hobsons Bay City Council

  • Development of policies and procedures to support community care operations and preparations for the CCCS audit
  • Evaluation of progress towards ASM implementation, years 1 and 3.

 

Collaborative projects


Hume Region

  • Strengthening collaborative practice across HACC organisations, including practitioner workshops and protocol development supporting practice.
  • ASM Alliance implementation plan, including partnerships, staff development and governance for the Department of Health.

Municipal Association of Victoria

Demonstrating trends and changes in the delivery of HACC ASM services through the Home Support Worker role.

Development of the Northern and Western Metropolitan Community Care Alliances 

This included extensive consultation with the sector and project management through transition.

Loddon Mallee Community Care Alliance

Facilitation of the development of this Alliance.
